Radiologic Atypical Fibroxanthoma Definition Cellular cutaneous mesenchymal neoplasm with at least focal pleomorphism and lacking evidence of epithelial or melanocytic differentiation Atypical fibroxanthoma (AFX) is a dermal spindle-cell tumour that typically occurs on the head and neck of sun damaged older people. The tumour-like growth should be considered a type of skin cancer but it may behave in a benign fashion. A rare type of atypical fibroxanthoma occurs in younger patients on parts of the body that are not normally overexposed to the sun. Atypical fibroxanthoma is a malignant skin tumor with histologic features similar to those of undifferentiated pleomorphic sarcoma, but lacking its more aggressive behavior.

Atypical fibroxanthoma (AFX) is an uncommon, pleomorphic, spindle cell cutaneous malignancy that most commonly presents as a solitary red or pink papule or nodule on the head or neck (picture 1A) [ 1 ]. Exposure to ultraviolet light most likely contributes to the development of AFX.
